There are two times per year when benefits are not deducted, specifically when there are three pay dates in the same month.\u00a0<\/span><\/i><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Qualifying_Life_Event\"><\/span>Qualifying Life Event<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The benefit choices you make will remain in effect for the entire calendar year unless you experience a qualifying life event. You must complete the enrollment form within 30 days of the event. You will need to provide documentation that proves the type of change and the date of change.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The Qualifying Life Event form will be sent to Human Resources for review and approval. This does NOT change your benefits.<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">Once you have received the approval from human resources, complete the <\/span><a href=\"https:\/\/forms.augsburg.edu\/frevvo\/web\/tn\/augsburg.edu\/u\/9b4c474e-f04d-4d8c-93a1-6eae144d03aa\/app\/_94HOQcpLEeyHxb_e1xqoVw\/flowtype\/_02YnsMpOEeyHxb_e1xqoVw?_method=post&amp;embed=true\"><span style=\"font-weight: 400;\">Benefits Enrollment\/Change form<\/span><\/a><span style=\"font-weight: 400;\"> to complete your benefits. If a Benefits Enrollment\/Change form is not completed, your benefits will not change. <\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Insurance_Cards_Benefit_Account_Creation\"><\/span>Insurance Cards\/ Benefit Account Creation<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>All Insurance Cards (Medical, Dental, Vision) will be mailed to the address on file within 7-14 days. If you have not received a card during that time, you must call the carrier directly. Contact information is listed under each benefit heading. Your insurance will be retroactive to your benefits&#8217; effective date, even if you have not received cards yet.<\/p>\n<p><strong>Medica<\/strong>\u00a0offers account creation, available 7 days after Medica receives your information. The account creation offers members the ability to review insurance cards and access important insurance information.<\/p>\n<ol>\n<li>To create an account, go to\u00a0<a href=\"http:\/\/medica.com\/SignIn\" target=\"_blank\" rel=\"noopener\" data-saferedirecturl=\"https:\/\/www.google.com\/url?q=http:\/\/medica.com\/SignIn&amp;source=gmail&amp;ust=1749914598080000&amp;usg=AOvVaw34WhZYn2NmISZMR8it3pe_\">Medica.com\/SignIn<\/a>.<\/li>\n<li>Select &#8220;Create account&#8221; and follow the instructions to create an account.<\/li>\n<li>You will be asked to log in with your email(Augsburg email or personal email is fine) and your membership with your Medica ID number (found on your Medica ID card) or a partial social security number. If you have not received your Medica card in the mail, use your Social Security number.<\/li>\n<\/ol>\n<p><strong>Chard Snyder<\/strong>\u00a0(for Health Savings and all Flexible Spending Accounts) will mail a debit card to all enrolled employees. The debit card can take up to 14 days to be sent. An email will also be sent to your Augsburg email once your information has been received by Chard Snyder (this typically occurs a few days after you submit your enrollment form, excluding open enrollment). You will be able to create an account with Chard Snyder after that time, where you will be able to view your balance, pay, and manage your funds.<\/p>\n<p><strong>Kavira<\/strong>: You will receive a message to your Augsburg email when your information has been entered in the Kavira Website.<\/p>\n<p><strong>TIAA (retirement)<\/strong>: After the first pay date when your retirement contribution occurs, you will be able to log in to your retirement website to make allocations and manage your funds.
