through Human Resources.\n<ul>\n<li><i><span style=\"font-weight: 400;\">This form notifies HR of your leave request and decision to \u201ctop off\u201d your leave with any available sick and vacation time (sick leave will be used first, then vacation if applicable).<\/span><\/i><\/li>\n<\/ul>\n<\/li>\n<li><a href=\"https:\/\/www.unum.com\/employees\/file-a-claim\">File a Claim through Unum<\/a>, our approved third-party insurer.\n<ul>\n<li><em>Do not apply through the state of MN for your leave from Augsburg<\/em><\/li>\n<li>Apply using your personal e-mail address<\/li>\n<li>Watch for updates and additional requests from Unum regarding your leave<\/li>\n<li><i><span style=\"font-weight: 400;\">Unum tracks and approves your leave in accordance with MNPFML guidelines. This can be done online, by fax, or by phone through Unum.<\/span><\/i><\/li>\n<li>Don&#8217;t want to file online? <a href=\"https:\/\/web.augsburg.edu\/hr\/MNPFML\/PFMLClaimFormUnum.PDF\">Printable Unum PFML Claim Form (PDF)<\/a><\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<h2><span class=\"ez-toc-section\" id=\"Who_is_Covered\"><\/span>Who is Covered?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<ul>\n<li>All employees who\n<ul>\n<li>work at least 50% in Minnesota and<\/li>\n<li>earn over 5.3% the statewide average yearly wage ($3,900 as of November 2025)<\/li>\n<\/ul>\n<\/li>\n<li>This includes faculty, staff, adjunct faculty, and student employees<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Types_of_Leave\"><\/span>Types of Leave<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>All leaves are subject to Unum&#8217;s approval. Qualifying leaves lasting seven (7) or more days for the following reasons are covered under MN PFML :<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Medical_Leave\"><\/span>Medical Leave<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>Take care of yourself for a serious health condition.<\/li>\n<li>A serious health condition means a physical or mental illness, injury, impairment, condition, or substance use disorder.<\/li>\n<li>Taking care of yourself for this serious condition may involve evaluation, treatment, inpatient care, recovery, or not being able to perform regular work, attend school, or do regular daily activities.<\/li>\n<li>This includes childbirth, conditions related to pregnancy, or surgery.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Family_Leave\"><\/span>Family Leave<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>Bonding leave \u2013 to care for and bond with a new child welcomed through birth, adoption, or foster placement<\/li>\n<li>Caring leave \u2013 to care for a family member with a serious health condition<\/li>\n<li>Military family leave \u2013 to support a family member called to active duty<\/li>\n<li>Safety leave \u2013 to respond to issues such as domestic violence, sexual assault, or stalking.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"What_Are_the_Benefits\"><\/span>What Are the Benefits?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<ul>\n<li>Up to 20 weeks of combined leave per benefit year\n<ul>\n<li>Up to 12 weeks of medical leave<\/li>\n<li>Up to 12 weeks of family leave<\/li>\n<li>A benefit year is a 52-week period that starts on the first day an employee is absent from work for a qualified reason.<\/li>\n<\/ul>\n<\/li>\n<li>While on leave\n<ul>\n<li>Job protection<\/li>\n<li>Insurance continuation<\/li>\n<li>Partial wage replacement (<a href=\"https:\/\/mn.gov\/deed\/paidleave\/employees\/leave-time\/\">PFML Benefits Estimate Calculator<\/a>)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Who_Pays_for_this_Benefit\"><\/span>Who Pays for this Benefit?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Employees and their employers share the cost of the benefit premiums.<\/p>\n<ul>\n<li>Augsburg will pay at least 50% of premiums<\/li>\n<li>Employees pay 0.44% of wages as a post-tax deduction<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Claim_Process_Through_Unum\"><\/span>Claim Process Through Unum<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There are generally three steps needed for your claim to be approved.\u00a0<\/span><\/p>\n<ol>\n<li><b>File a claim through Unum.<\/b><span style=\"font-weight: 400;\"> This starts the claims process and requests more details about your leave. You can state if you want a continuous, intermittent, or reduced schedule leave.\u00a0<\/span><\/li>\n<li><b>Submit a healthcare provider certification<\/b><span style=\"font-weight: 400;\">. The easiest way to accomplish this is by using the <\/span><a href=\"https:\/\/web.augsburg.edu\/hr\/MNPFML\/PFMLClaimFormUnum.PDF\"><span style=\"font-weight: 400;\">Unum PFML Claim Form<\/span><\/a><span style=\"font-weight: 400;\">. Pages 6 through 8 will go to your healthcare provider. Unum will also reach out to your provider for this information.<\/span>\n<ul>\n<li><i><span style=\"font-weight: 400;\">Ensure that the dates of leave are communicated to your healthcare provider. If the dates you requested leave do not match your healthcare provider certification, Unum may not approve your full requested leave.<\/span><\/i><\/li>\n<\/ul>\n<\/li>\n<li><b>The employer certification form.<\/b><span style=\"font-weight: 400;\"> Unum will send Augsburg the employer portion of the form to complete.<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">Unum estimates that a claim can take up to 5 days to be fully processed once all items are received. Claim approval may be delayed if information is missing or incomplete. If your leave has been submitted but not yet approved, and you have begun your leave, the University will provide up to two weeks of pay based on anticipated leave payroll payments.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Unum will approve or deny your claim based on the information provided. Augsburg will use the leave dates approved by Unum. You are expected to return to work on the date that is specified by Unum. If you have remaining MNPFML hours and need to extend your leave, you must contact Unum to request an extension and notify the benefits team. Time off that extends beyond the dates specified by Unum is considered unapproved. If you are taking medical leave, you may be eligible for short-term disability after your PFML leave is exhausted.<\/span><\/p>\n<p><strong>FACULTY: <span style=\"font-weight: 400;\">You may elect to use available sick time to extend your leave until the end of the semester. Please be advised that you may have unpaid time if your sick time reaches zero while on leave. <\/span><\/strong><\/p>\n<h2><span class=\"ez-toc-section\" id=\"Wage_%E2%80%9CTop_Off%E2%80%9D\"><\/span>Wage &#8220;Top Off&#8221;<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Employees may top off their PFML benefits using accrued sick and vacation time. And if you are a birthing parent who needs time off to care for your health during pregnancy or after giving birth, you may be eligible for Medical Leave, too.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Birth_Parents\"><\/span>Birth Parents<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Medical Leave may be taken to recover after delivery, subject to your PFML medical leave balance. Six (6) weeks is standard, but you may receive up to eight (8) weeks depending on the delivery method and complications.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You may request up to 12 weeks to bond with your newborn, up to their first birthday. Bonding leave does not need to be taken immediately after medical leave. You may take the leave continuously in one block, in multiple blocks, or with a reduced work schedule. Unum determines the intermittent or reduced schedule leave based on your claim.\u00a0<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Staff\"><\/span>Staff<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\">While on intermittent leave, you will continue to manage your timesheet and will indicate the days you are absent due to a medical\/ family leave event, which payroll will then review and adjust with a top-off if elected. Augsburg approves leave in half-day increments, so you will record either four (40 hours or eight (8) hours of \u201cMedical\/Family Leave Request\u201d on your timesheet, regardless of whether you are an hourly or salaried employee.<\/span><\/p>\n<h3><span class=\"ez-toc-section\" id=\"Faculty\"><\/span>Faculty<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span style=\"font-weight: 400;\">If you take leave on a reduced schedule, you are expected to scale your scholarship and service at the same rate as your teaching. The reduced-schedule leave calculations are based on the reduced course load and a 40-hour full-time workweek (and yes, we know that isn&#8217;t always the reality). Our internal policy requires taking leave in 4-hour increments. You will use paid leave during the weeks of the academic term, including fall or spring break, <\/span><span style=\"font-weight: 400;\">but it will not apply to breaks between terms.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you are looking for a one-course release, you would request 12 hours of a reduced schedule per week.<\/span><\/p>\n<p><b>Calculation<\/b><span style=\"font-weight: 400;\">:<\/span><\/p>\n<p><span style=\"font-weight: 400;\">1\/3 * 40 = 13.333.
