Research has shown that forms with multiple targets are less useful.\u00a0 The form was revised by a <a href=\"http:\/\/sites.augsburg.edu\/ctl\/resources\/augsburg-resources-and-support\/course-evaluations\/working-group-2017-2018\/\">working group in 2017-2018<\/a> and approved by the faculty in 2018.<\/p>\n<p><a href=\"https:\/\/web.augsburg.edu\/it\/evals\/UCS-last-draft.pdf\">Sample of current form<\/a> (2018 &#8211; )<\/p>\n<p><a href=\"http:\/\/sites.augsburg.edu\/ctl\/files\/2012\/08\/sample-evalform.pdf\">Sample of the past form<\/a>\u00a0(2005-2017)<\/p>\n<h2><strong>Process<\/strong><\/h2>\n<ul>\n<li>University course surveys are delivered online to all courses having 3 or more students with some exceptions.<\/li>\n<li>All courses are surveyed every term.
